The Chiropractic Wellness Center and the medical practice of Dr. Richard Ranallo has closed due to the unexpected illness of Dr. Ranallo. This notice provides information to patients of the Chiropractic Wellness Center, Inc. and Dr. Ranallo regarding the status of their medical records. Upon the written request of the patient or the patient's personal representative, all patients are entitled to receive a copy of their medical records or to request that their records be forwarded to another licensed Chiropractor or other licensed provider of the patient's choice.
