On August 1, 2017, ProCare Physical Therapy, LLC will be transferring its medical records to PT Network, LLC, d.b.a. Pivot Physical Therapy or one of its subsidiaries. As required by Virginia law, ProCare Physical Therapy, LLC is required to inform current and prior patients that if they or an authorized representative would feel the need to transfer their records to another physical therapy provider, or if they would like a copy of their physical therapy records, they may do so by sending a written request to the office with the address where they would like to have the records sent. ProCare Physical Therapy, LLC is also required to tell patients there will be a charge of 50 cents for the first page and then 25 cents for each additional page plus mailing fees to process the request.
